The Homeless Prevention Center (HPC) seeks a dynamic individual to provide housing case management and service coordination to families in our innovative Family Supportive Housing program who are transitioning from homelessness or rental instability to stable housing. A Service Coordinator works collaboratively with families and our community partners to: provide resources and support to clients; assist families in developing life skills; facilitate access to permanent housing; build financial literacy and support stable tenancies. Principal Duties: Provide support to between 12 to 15 client households on a caseload. Meet regularly with clients (both inside and outside of HPC offices) to assess housing needs and complete intake, following grant guidelines. Provide training and referrals to clients in the areas of financial empowerment, life skills, tenant education, parent and child resiliency, and support in addiction recovery. Assist clients in preparation of individualized housing plans to determine their goals and assist with self-advocacy in relation to housing and family domains. Engage in advocacy and collaboration with agencies on behalf of families. Coordinate various plans among appropriate agencies, particularly with DCF Family Services and Reach-Up, and conduct general advocacy with a focus on obtaining permanent and safe housing for clients and their children. Identify and access additional appropriate resources for families. Attend required trainings and be available for additional training opportunities as identified by the Program Supervisor or Executive Director. Perform other duties as specified under the Family Supportive Housing grant. Administrative Duties: Maintain appropriate records and client case notes as required. Work with partners such as NewStory Center in general support of issues surrounding DV and SV. Perform additional administrative or other duties as needed. Maintain time and mileage records Qualifications: A degree in Human Services, Social Work, Education, Counseling, Family or Child Development or related field is preferred, with strong case management experience working in human services, particularly with families and/or people who have experienced homelessness. Knowledge and experience working with people struggling with poverty and substance abuse. Knowledge of resources in the community, along with the ability to teach basic life skills, including budgeting and personal safety skills. Terms of Employment: This is a full-time position with benefits offered by HPC, including paid vacation and personal time, paid sick time, paid holidays and a voluntary SIMPLE IRA plan. The position requires driving of a personal vehicle which is eligible for mileage reimbursement at the prevailing rate. This individual will be hired by the Executive Director, supervised by the Program Supervisor and be subject to an initial three-month probationary period.
