About the Role:
The Manager of Intensive Supports is responsible for overseeing and coordinating services for individuals with developmental disabilities who require intensive, 24/7 care and/or have complex needs. This role focuses on providing leadership, planning, and supervision of support services for individuals, including those receiving supervised living, staffed living, and in-home support, including those under ACT 248. The Manager of Intensive Supports ensures individuals receive the highest quality care in the least restrictive environment, working collaboratively with internal and external teams to meet their unique needs.
Principal Responsibilities:
- Collaborate with teams within Developmental Services (DS), Community Care Network (aka. CCN), and external agencies to ensure the needs of individuals are met effectively and efficiently.
- Coordinate appropriate staff coverage and develop staff schedules to ensure strong continuity of services and maintenance of productivity targets.
- Uphold and promote CCN values throughout the Intensive Supports Team to ensure high-quality, person-centered care.
- Ensure all documentation is completed in a timely and accurate manner, in compliance with the Department of Disabilities, Aging and Independent Living (DAIL), CCN, and Medicaid guidelines.
- Participate in the DS On-Call rotation for 24/7 crisis support as part of the DS Leadership Team.
Qualifications:
- Bachelor’s degree in related field preferred; equivalent experience will be considered.
- 4 years minimum experience in developmental disabilities services, with experience in intensive supports or similar services preferred.
- Eligible for Qualified Developmental Disabilities Professional (QDDP) status.
- Previous leadership or management experience required, with strong leadership skills, including accountability, follow through, coaching techniques, constructive feedback, and multitasking.
- Proficiency with computers, experience entering data into electronic medical records (EMR) program.
- Ability to work with others in a positive, responsible manner.
- Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
- Flexibility, dependability, and ability to work independently.
- Ability to respond in emergency situations appropriately and ability to adjust work schedule as needed to meet program demands.
- Commitment to person centered services, community inclusion and individual choice.
- Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
